Child & Family Clinic Plus

Child and Family Clinic Plus is an early recognition and intervention program funded by the New York State Office of Mental Health and operated at a local level by MHPWQ, Inc.Child and Family Clinic Plus extend clinic services into the community by providing clinical services in the school setting and in the home environment.

 
Western Queens Blended Case Management
Mental Health Providers of Western Queens operates a Blended Case Management Program which is funded and regulated by the New York state Office of Mental Health. The program provides case management services to adults, adolescents and children in the queens community who are living with mental illness and/or serious emotional problems. Recipients are provided with either an Intensive Case Manager (IMC) or a Supportive Case Manager (SCM) dependent on recipient need and OMH criterial. 
 
Western Queens Recovery Services
Western Queens Recovery Services A comprehensive, family oriented bilingual Spanish/English outpatient treatment facility that treats Chemically Dependent Individuals and Significant Others to help in their start to a healthy substance-free lives. WQRS is a community-based program licensed by New York State Office of Alcoholism and Substance Abuse Services (OASAS).
 
 
Families First Program
A community outreach, parent education program at Western Queens Consultation Center
which offers workshops and training sessions in the community and at our program sites.

ServicesEligibility Criteria
Mental Health Providers of Western Queens operates a Blended Case Management Program at our Sunnyside office.  This Case Management program is funded and regulated by the New York State Office of Mental Health.  The program provides case management services to adults, adolescents and children in the Queens community who are living with mental illness and/or serous emotional problems.  Recipients are provided with either an Intensive Case Manager (ICM) or a Supportive Case Manager (SCM) dependent on recipient need and OMH criteria.  The recipient and the case manager work together as a team with the goal of helping the client to become more independent/self sufficient and a more active member of the community at large.  In the case of children, case managers work with the entire family. Case manager meet with recipients in their place of residence, at other social service agencies and other public venues.  Case managers are available for crisis intervention 24 hours per day, seven days per week. 
 




Services

  • Assessment and referral
  • Weekly (ICM)/bi-weekly (SCM) face to face contact with the recipient in the community including a minimum of one home visit per month
  • Service planning that addresses stability, treatment compliance daily living skills, vocational, educational, social, housing and financial goals as well as other concrete services
  • Medication compliance monitoring
  • Accompaniment and assistance with appointments and daily living skills such as shopping
  • Coordination of services
  • Advocacy
  • Regular collateral contact including face to face visits with recipients and providers at day programs, clinic, etc. 
  • 24-on-call crisis services

Eligibility Criteria


Adults
  • A diagnosable, chronic, persistent mental illness (Axis I Diagnosis)
  • 18 years of age or older
  • Reside in the Borough of Queens
  • Utilize emergency/crisis services frequently and/or is homeless
  • Some impairment in ability to function independently, appropriately and effectively
Adolescents / Children
  • History of severe/persistent psychiatric problems (Axis I diagnosis)
  • 5-18 years of age
  • Reside in the Borough of Queens
  • May be at risk for hospitalization or re-hospitalization and/or problematic/chaotic home environment
